We would like you to join us in our journey to improve our services so we are inviting patients to join our Patient Participation Group.  The group will be asked to complete a series of surveys from time to time on the services the practice offers such as opening times or the quality of care/service you have received.  Our surveys will be concise so should not take up too much of your time. 

If you would like to take part, please download the form from here and send back to us either via email, post or drop it in at reception.  All information you supply us with will be used lawfully, in accordance with the Data Protection Action 1998. 

We need a broad cross-section of patients in our group, young people, senior citizens, people with long term conditions, people from different ethnic groups, men and women.
